Qualtrics User Access
This Qualtrics license is for specific departments within the University of Utah administration and not for all students, faculty, and staff. If you are not within one of these areas, go to utah.qualtrics.com to create your account.
Who needs access? Not everyone needs access to Qualtrics; individuals can preview a project and enter data without an account. Assessment Liaisons can even export pdfs of results to share with stakeholders. Please limit access requests to those who will be editing survey questions, analyzing results, presenting data within your department, etc.
New User Access: For potential new users requesting access to this license of Qualtrics:
- Sign the online data agreement:
- Student Affairs staff

- If within Student Affairs, director will be notified to sign off on the new account via a second electronic form. Note that liaison access will only be granted after training.
- Make sure you have completed FERPA Certification, which is available on the website of the Office of the Registrar.

Log-in information: Your username is in the format unid@utah.edu, and you can reset your password from the login screen.
